We Know Care
Claire Hieber, LCSW
Claire Hieber is the Chief Care Officer at Sycamore Care Management and the central point of contact for client care. She earned her degree from the Jane Addams College of Social Work at the University of Illinois at Chicago and is a licensed clinical social worker with more than two decades of experience in care coordination and mental health.
Claire has worked extensively with older adults, as well as younger clients living with chronic mental illness or under conservatorship—helping them access resources, navigate systems, and maintain stability in daily life. She also brings valuable experience in hospice and end-of-life care, ensuring individuals and families receive thoughtful support during critical transitions. Her work is grounded in a practical, client-centered approach that emphasizes personalized planning, independence, and continuity of care.
Claire is a longtime member of the Aging Life Care Association (ALCA) and remains actively engaged in the field of aging and disability services. She takes pride in building strong relationships with clients and families, helping them navigate complex decisions with clarity and compassion.
